Our Mental Health Care Package is design to provide care and support to individuals with mental health conditions within the comfort of their own homes. Our aims is to improve the mental well-being and overall quality of life of individuals dealing with various mental health challenges.
Here are some key aspects of our homecare services for individuals suffering from mental health conditions:
Dedicated Specialist Personal Care Assistants:
Our team of Specialist Personal Care Assistants have received advance training in caring for individuals with mental health condition and can offer emotional support to both the individual and their relatives. This can help improve coping skills, reduce stigma, and enhance understanding of the mental health condition.
Personalise Mental Health Care Plan:
The process typically begins with a thorough assessment of the individual’s mental health needs carried out by one of our Area Community Managers. A Person-Centred Care Plan is then developed, which outlines specific goals, interventions, and the frequency of care visits.
Assistance with Activities of Daily Living (ADLs):
Depending on the severity of the mental health condition, Our Specialist Personal Care Assistants can assist individuals with daily tasks such as personal hygiene, meal preparation, and housekeeping.
Medication Management:
For individuals prescribed psychiatric medications, Our Specialist Personal Care Assistants will assist with medication management. This may include medication administration, education about the medications prescribed, and monitoring for side effects.
Support and Education:
Our Specialist Personal Care Assistants are trained to offer emotional support and education to both the individual and their family members. This can help improve coping skills, reduce stigma, and enhance understanding of the mental health condition.
Coordination of Care:
We work in collaboration with other Community Based Healthcare Professionals and community resources, ensuring a comprehensive approach to Mental Health care.
Therapeutic Interventions:
We will arrange and facilitate the involvement of therapists, including psychologists and counsellors (if required) in the individual’s own home. This may include cognitive-behavioural therapy, dialectical behaviour therapy, family therapy, and other evidence-based approaches.
Crisis Intervention:
Our Specialist Personal Care Assistants are trained to handle mental health crises. They can assist in de-escalating situations, ensuring safety, and helping individuals access appropriate crisis services when needed.
Social and Recreational Activities:
We encourage social engagement and recreational activities is essential for mental well-being. Homecare providers can plan and engage individuals in activities that promote social interaction and leisure.
Safety Measures:
We will regularly assess the individual’s home environment for safety concerns and implement measures to reduce potential risks to them.
Our Mental Health Care Package can be a valuable option for individuals who prefer to receive treatment and support in the familiar surroundings of their homes. It can also be a way to provide continuity of care after discharge from a hospital or psychiatric facility.
